In this “Inside-Baseball” seminar, former astronaut/ISS Commander Terry Virts guides participants through the fatal Space Shuttle Challenger (1986) and Columbia (2003) disasters, focusing on the mistakes that NASA program managers made and emphasizing the striking similarities between these two accidents, despite being separated by 17 years. In both cases, NASA management ignored warning signs from junior engineers and from the actual Shuttles and succumbed to severe schedule and budget pressures from HQ. This seminar explores risk management, 360-degree leadership, the importance of debriefing and feedback, and group decision-making techniques.
